So it sounds like their assumption, until proven otherwise, is that past withdrawal from your FSA needs to be returned, with the money coming from you (not the doctor). They send me insurance claim forms, with the doctors stamp and a HS code for a bundled rate. So I asked them for receipts again, with what is needed. The receipts i got didn't include the description of what the service is just that i had it charged and it had the doctors office on the letter head. Its a smaller doctors office, for medication, labs and office visits. My FSA company gave me a credit card to use, every time it goes through, no problem.Ħ months later I get an email that they need receipts to prove to the irs that its eligible.
